Although you do not own your medical records, you are entitled to have access to these and to have copies of them. If you wish to see your medical records, please contact the Practice Manager to arrange an appointment. For security purposes, we will require a signed request from you before you view your records.
Please be assured that, if a third party e.g. your solicitor submits a request on your behalf, we will not release any information without your written consent.
Confidentiality
In order to protect confidentiality, we ask you to collect any copies of records that you requested in person if possible. If we are required to post these, they will be sent by recorded delivery. This means that a signature will be required at the point of delivery.
